Fee Waiver Request

Complete this form to request that the University of Utah Human Research Protection Program (HRPP) consider waiving required fees for service. Waivers are considered on a case-by-case basis. When you submit this form, your request will be sent to the University of Utah Human Research Protection Program (HRPP). We will contact you if we need more information or when a determination has been made. You will be notified of our decision in writing.

After 3-5 business days, you may check on the status of your request by sending an email to the IRB at irb@hsc.utah.edu.
