All studies conducted at Primary Children's Hospital (PCH) are required to undergo a facilities review so Primary Children's can confirm that your study:

1. Aligns with Primary Children's Mission, Vision and Values
2. Receives approval from the Primary Children's resources impacted (e.g.: lab, radiology, infant unit, etc.); and
3. Identifies Primary Children's study related charges from standard of care charges

Additionally, all studies requesting a HIPAA waiver of authorization, use of a Limited Data Set, or use of a de-identified dataset for use of PCH data must receive Privacy Board Review by the Intermountain Privacy Board.  Consent and authorization documents used at PCH are not considered by the Intermountain Privacy Board, but are approved by the University of Utah IRB.  

What should I submit? In the IRB New Study Application, check "Primary Children's Hospital (PCH)" on the "Study Location and Sponsors" Page.  The PCH Administrative Research Questions Form will automatically generate as you click "Continue" through the application.
